Michigan Collaborative: Three Competing Health Systems Partner to Improve Care

Improving care transitions requires the willingness of healthcare organizations and post-acute care providers to work across the continuum. In Detroit, three competing healthcare organizations and the state quality improvement organization joined together to create a strategy to address readmissions from skilled nursing facilities. Learn how they standardized reported metrics, created a regional dashboard, and identified improvement opportunities that were common across all organizations.
