Patient-Centered Perspective on Readmissions at UCLA Health

Nasim Afsar, MD, SFHM, Associate CMO and Assistant Clinical Professor, UCLA Hospitals. Two years ago, Dr. Afsar and her team embarked on a journey to understand readmissions from the patient perspective. This has led to a number of interesting findings about why patients return to the hospital. This presentation reviews these findings and discusses the next steps to address the multi-factorial solutions to reduce readmissions.
