Ochsner Health has taken to heart its commitment to improve the health status of the communities it serves. Their vision is bold – to advance Louisiana’s state health ranking to the top 40 by 2030 in America’s Health Rankings – and their approach is profound – to enlist all the stakeholders across the state in making this goal a reality. Louisiana’s Healthy State by 2030 initiative is a working example of how a health system can effectively utilize a collective impact model to improve the health ranking of a state. Healthy State by 2030 is driven by a comprehensive strategy to enhance healthcare access\, improve health equity and health outcomes\, by partnering with business leaders\, educational institutions\, community members and others\, coordinated through a statewide Healthy State advisory board and a network of local community councils. Together\, these community resources are enacting plans to reduce child poverty\, smoking\, obesity\, low birth weights\, cardiovascular and cancer deaths\, as well as chronic conditions such as diabetes\, high blood pressure and COPD\, and to address non-clinical issues that impact public health\, including income inequality\, food insecurity\, workforce development and broadband access.
