DESCRIPTION:Chris Belmont\, MBA\, VP & CIO\, Memorial Hospital at Gulfport\, presented with Wolters Kluwer\, Health.  Memorial Hospital at Gulfport was always interested in finding ways to reduce readmission rates and connect patients with appropriate follow-up care to improve wellness and lower unnecessary costs. Chris found that real results started to emerge when the hospital stopped looking at patient engagement as individual targeted activities and began to invest in a strategic\, site-wide engagement initiative and partnership with patients.
