DESCRIPTION:Nabil Chehade\, MD\, MSBS\, Interim Chief Executive Officer; Executive Vice President\, Chief Population and Digital Health; James Misak\, MD\, Assistant Professor\, Institute for H.O.P.E.; and Kevin Chagin\, MS\, Manager\, Advance Analytics & Data Operations\, Institute for H.O.P.E.\, The MetroHealth System. The Greater Cleveland area has some of the best health care institutions in the world. Yet it also has some of the worst health outcomes across its population. In 2022\, Cuyahoga County ranked 11th\, out of 88 counties\, for clinical care and 82nd in quality of life and social factors. Across two local ZIP codes\, just five miles apart\, residents have a 23-year difference in their life expectancy. In 2019\, The MetroHealth System’s Institute for Health\, Opportunity\, Partnership and Empowerment (H.O.P.E.TM) was founded to identify social drivers of health (SDOH) within the MetroHealth patient population and to address health-related social needs. \nMetroHealth has developed an approach to understand the SDOH impact on their patient population\, e.g.\, who is at risk\, what risks are being experienced\, and how these risks impact medical comorbidities\, treatments\, and hospital utilization\, starting with a screening process organized through their Institute for H.O.P.E.TM and using a variety of means: community health workers\, social workers\, and care coordinators; in person or via a paper-based or electronic interface at the point of care; through secure online health management tools (MyChart); and at hospital events (i.e.\, COVID vaccine clinics and the Minority Men’s Health Fair). These screens were then merged and connected to patient demographics\, geospatial location\, healthcare characteristics (primary care location\, payer status\, etc.)\, comorbidities\, clinical and laboratory data\, hospital utilization data (ED visits\, hospitalizations\, appointment no shows\, etc.) and publicly available data (US Census)\, and a dashboard was created to highlight key findings and tell the story of how SDOH impact the community. Dr. Weiss reviews the purpose of physician notes; demonstrates how there are disagreements about EHR note quality and show how quality can be judged using proven tools; discusses the advantages and disadvantages of Copy and Paste; and illustrates the dangers of note bloat and its causes.

Funded by a $1.5-million grant from the American Nurses Foundation\, Wilmington\, Del.-based ChristianaCare is the first health system in the Philadelphia region piloting an innovative tool called “Moxi\,” a collaborative robot—or “cobot”—that can assist in the hospital by making deliveries\, retrieving equipment and supplies and performing other non-clinical tasks so nurses and other clinical staff can spend more time caring for patients. Moxi cobots will be integrated with the EHR as part of the grant project and use AI to proactively identify when nurses will need equipment\, supplies\, medications and lab tests. Three Moxi cobots are deployed to 11 inpatient units\, partnering with more than 400 nurses\, completing 200 tasks a day\, saving nurses thousands of steps.

DESCRIPTION:Robert L. Trestman\, PhD\, MD\, Professor and Chair of Psychiatry and Behavioral Medicine\, Institutional Research Officer\, and Co-Chair of the Addiction Taskforce\, Carilion Clinic. Carilion Clinic spans 250 miles of rural Virginia. In 2017\, they faced an exploding epidemic of opioid misuse\, addiction\, overdose\, and death. They saw it in the emergency departments\, primary care offices\, operating rooms\, and inpatient units. Many isolated projects were already developed by individuals and small groups to address elements of this epidemic. In the fall of 2017\, Carilion leadership chartered the Carilion Opioid Taskforce to develop and coordinate an orchestrated systemic response to these severe challenges. Since then\, Carilion has transformed its approaches to prevention\, management\, treatment\, and recovery from opioids. This has led to innovative collaborations and a genuine transformation in the organization’s ability to coordinate and deliver needed care.
