SUMMARY:WEBINAR | The Transformative Power of Systems-Thinking in Population Health Management
DESCRIPTION:Holly Urban\, MD\, MBA\, Vice President\, Business Development-Strategy\, Clinical Effectiveness\, Wolters Kluwer Health. The traditional primary care model often lacks a proactive focus\, especially in adult care\, which tends to be reactive rather than preventative. This approach results in fragmented care\, higher rates of chronic conditions\, undetected diseases\, and increased reliance on emergency services\, ultimately placing a financial strain on the healthcare system. Pediatric care\, conversely\, is inherently proactive\, focusing on growth\, immunizations\, and preventive measures.  \n  \nThere is a transformative potential for applying a systems-thinking approach to population health management and primary care. This webinar highlights how adopting this model can lead to improved health outcomes\, drive reduced healthcare costs\, and address health disparities.  \nAfter practicing as a primary care pediatrician\, Dr. Urban worked for several EHR technology and evidence-based content companies\, and has served in healthcare IT leadership roles for over fifteen years. With this experience\, she brings unique insight into how evidence and workflow can optimally merge to benefit clinical users and healthcare teams. Her experience with both provider and payer customers allows her to holistically understand the healthcare ecosystem with all the complicated drivers that influence how care is provided.

SUMMARY:FIRESIDE CHAT | Amy Compton Phillips and Jim Dunn: Simplify Healthcare and Increase Patient and Care Team Satisfaction
DESCRIPTION:Technology is transforming how healthcare is delivered at a previously unimagined pace. Keeping up with this change is creating additional stress in the already complicated experience of providing healthcare or using it. \nWe cannot ask any more of our exhausted caregivers and we certainly can’t ask patients to work harder on their own to navigate the system towards health. Amy Compton-Philips\, MD\, President and Chief Physician Executive\, Press Ganey shares how\, with these same new technologies\, health systems now have the ability to radically improve these experiences. With ambient listening\, AI assistants\, automation and analytics\, we can change the way we work in constructive ways. And\, patients are now able to monitor themselves\, access help and receive care at home. Hear about ways in which health systems are identifying and reducing unnecessary friction\, eliminating unnecessary tasks\, streamlining communication and simplifying repetitive work. And\, by doing so\, they are also re-humanizing healthcare.

SUMMARY:WEBINAR | Sentara Health’s Centralized Home Health Care Delivery Model
DESCRIPTION:As patient care moves more assertively into people’s homes\, the models of care are evolving clinically and operationally. Sherry Kesler\, Director of Home Health Services at Sentara Health shares how they developed a centralized case management practice to see more patients and support clinical staff working at top of their license. The new model addresses the inefficiencies in traditional care models that utilize clinical staff to perform non-clinical functions and require clinicians to case manage between visits. Also learn the role of technology supporting this new model of care. \nSponsored by Epic.

SUMMARY:WEBINAR | Saving Lives and Reducing Stress in the ICU at Houston Methodist: Predicting the future can help!
DESCRIPTION:Faisal Masud\, MD\, FCCP\, FCCM\, Houston Methodist Chair in Anesthesiology & Critical Care and Medical Director\, Center for Critical Care\, Houston Methodist Hospital presents the Cardiac Arrest Prediction Model (CAP-M)\, an Augmented Intelligence (AI) tool. \nAn in-hospital cardiac arrest (IHCA) code blue affects nearly 290\,000 hospitalized patients each year in the United States and is associated with poor survival and a high risk of neurological disability among survivors. Survival rates for IHCA have plateaued in recent years\, with a mean survival rate of approximately 25%. That means 3 out of 4 will not make it. \nWhat if we could predict In-Hospital Cardiac Arrest (IHCA) in an ICU patient? That is the question the Houston Methodist team asked themselves when they started their journey in developing the Cardiac Arrest Prediction Model (CAP-M). Critical Care patients are the most monitored patients in hospitals so this represents an opportunity to utilize the information to save lives\, improve outcomes and even reduce stress in the ICUs. \nDr. Masud discusses their goals\, methodologies\, and the progress made in developing a model that promises to be a key Augmented Intelligence (AI) tool that will play an important role in keeping the promise of delivering world class critical care at Houston Methodist through innovative thinking.
