Kate and Meadow present several action-oriented tools and guidance documents that hospital-community networks can use to put essential characteristics in place. The collaboration resulted in the co-creation of a standardized future state process leveraging emerging evidence and leading practices. The team successfully completed a system-wide implementation in all Emergency Departments\, Inpatient\, and Procedural areas. Responsibility assignment matrix (RACI) tools and ADKAR change management principles positively impacted adoption\, the people side of the change.
