Suicide is the 10th leading cause of death in the U.S. with an average of one suicide every 12.6 minutes. Indiana University Health, a 16-hospital system activated a team of nurses, providers, and other key stakeholders to assess the workflows for screening, assessment, intervention, and discharge of patients at risk for suicide. The collaboration resulted in the co-creation of a standardized future state process leveraging emerging evidence and leading practices. The team successfully completed a system-wide implementation in all Emergency Departments, Inpatient, and Procedural areas. Responsibility assignment matrix (RACI) tools and ADKAR change management principles positively impacted adoption, the people side of the change.
