November 2 – 3
In-Person at Westin Kierland, Scottsdale, AZ or Virtually

Sponsored by Epic

Make an impact on whole-person healthcare—diverse, inclusive, holistic—by joining forces with your peer experts in person OR virtually. Then, we invite you to return November 16-19 to enjoy SI’s 2021 Fall Symposium, Cracking the Code: Healthcare 2025.

The summit starts at 3pm MST (Mountain Standard Time) on Tuesday, and wraps up on Wednesday at 2pm MST. Virtual attendees will be provided with Zoom links prior to the Summit.

The Summit discussions will result in a white paper available to the industry through SI, Epic, and your own organizations, as desired. You’ll have an opportunity to review and edit your quotes before the report is finalized.
For more information and resources, see our SDoH Affinity Group page.
Attendance at the SDoH Summit is by invitation only. For info and invites, contact VP of  Executive Engagement Cynthia Schroers via email or fill out the form at the bottom of this page.
In-person attendees: TIME IS RUNNING OUT! Reserve your hotel room by October 8 for November 2-3 at the Westin Kierland. Space is limited.
