Michelle Mencke, Manager, Patient Education and Accessibility Services, Virtual Care Enterprise; Margie Diaz, MSN, RN, Director, Inclusion, Diversity and Health Equity; and Marielee Santiago, Director, Transformative Knowledge & Education Institute for H.O.P.E., The MetroHealth System.  Healthcare is faced with creating equity for all, which requires engaging health systems, patients, employees, and the greater community.  The MetroHealth System in Cleveland, Ohio, an essential health system, serves a majority of underserved and underrepresented individuals largely impacted by social determinants of health.
