Join us to learn how UW Health is reaching patients in the home with Home-Based Primary Care and Home-Based Hospital Care. Addressing social drivers of health is a key component of meeting patient needs.
This webinar addresses understanding the impacts of social determinants of health (SDOH) on health outcomes; learning strategies to overcome barriers in addressing SDOH and close the loop in addition to screening; and understanding how to optimize team-based care to address SDOH in home-based care.
