Advocate HealthCare has entered into a relationship with Blue Cross to develop a like organization we call AdvocateCare. We have a clinical infrastructure, as well as IT resources as part of the care management process. Understanding you can’t manage what you can’t measure, also requires us to have a strong tracking tool for patient outcomes. We share our story for others to better understand what it takes to be successful.
