Augusta Health’s mission is to strengthen the community they call home – Leigh knows this requires a strategic approach to improving health equity throughout the Shenandoah, Virginia region. Augusta Health’s efforts have earned national recognition, including the Health Equity Award from the Centers for Medicare and Medicaid Services and the Carolyn Boone Lewis Equity of Care Award from the American Hospital Association. Leigh shares how Augusta addresses its objective from several angles: leveraging the organization’s robust internal data and analytics program, expanding service access to under-served areas of the community, and developing collaborative partnerships with other healthcare providers and community service organizations. Highlights of their approach include in-depth geographical analysis fueling the operation of a Primary Mobile Care Clinic that services 14 distinct locations across the community each month, and a recent partnership with a women’s health clinic to extend care throughout the region.
