CMS and other payors are applying pressure on healthcare providers to reduce the number of 30-day readmissions for certain diagnoses. Learn how St. Vincent Health and its parent organization, Ascension, teamed together to sponsor a multi-hospital clinical trial to measure the impact of remote patient monitoring, video-conferencing with nurses and providing patient education/engagement tools virtually as part of a two-year Beacon Community Grant program (ARRA). Results and case studies are shared, along with lessons learned and how St. Vincent and Ascension are now establishing a Remote Care Management program based on their findings and cost avoidance ROI.
