Alan Weiss, MD, MBA, FACP, VP & CMIO, BayCare Health System There is controversy about whether electronic health records (EHRs) improve the quality of notes.  While some people believe that these notes are more complete, other find the notes harder to read.  Have we traded illegible hand-writing in the paper world for bloated and poorly formatted notes in the electronic one?  The purpose of this presentation is to explore studies and data on both sides of this issue as well as present solutions to improve note quality. Dr. Weiss reviews the purpose of physician notes; demonstrates how there are disagreements about EHR note quality and show how quality can be judged using proven tools; discusses the advantages and disadvantages of Copy and Paste; and illustrates the dangers of note bloat and its causes.
